What is CHSP?
The Commonwealth Home Support Programme (CHSP) is a scheme of the Australian Government that funds costs associated with getting help to keep living at home with age. The Scheme aims to empower older Australians by investing in their health and wellbeing and helping individuals who only require a low level of support with daily tasks to maintain their independence.
Getting started
In order to access CHSP and become a participant of the service there are some steps that need to be taken. Below is a quick summary of the initial process, full information can be accessed via the My Aged Care website or phone: 1800 200 422. CHSP Local Area Coordinators can walk you through the process and stages of getting started.

Contact My Aged Care (MAC) on 1800 200 422 to check your eligibility
A MAC representative will talk you through your application.
You will be assessed so you can get the right level of support
You will be supplied with the necessary forms for you and your health professional to complete.
Beginning your plan
Once your application is approved you can begin creating your first plan.
Your CHSP Representative or Local Area Coordinator will assist you with this.
Include any goals and services you think are necessary to help you achieve them.

Upon receiving your enquiry, our CHSP team will book you an appointment with a service provider suited to your needs. They will also talk you through the process of completing the required documentation including the Service Agreement. It will assist the process if you can have your CHSP registration number, Support Coordinator & Plan Manager details as applicable, and how your fund is being managed with you when you speak to our team.
